TO PARENTS OF CHILDREN WITH SPECIAL NEEDS AGES 3 THROUGH 21 YEARS: Your help is needed to improve the special education services in our school. This survey asks your opinion about the facilitation of parent involvement within your child's district. Each year the State must provide information in an Annual Performance Report (APR), indicating the State’s progress toward meeting the rigorous targets for each of the Indicators included in the State Performance Plan (SPP). One of those targets is Indicator 8 “Parent Involvement.” The Nebraska Parent Survey for Pre-School and School-Aged Children with Disabilities is the data collection method being used to gather information on opportunities provided to parents to participate in their child’s program and impact school improvement activities.
All responses are CONFIDENTIAL and demographic information collected is not for identification purposes. Demographic data is used for reporting to the US Department of Education to ensure responses are representative of the state of Nebraska in terms of race/ethnicity.
